Travel Permission Form

If you would like your student athlete to ride home with someone other than their own parent/guardian after a meet, please fill out the form on this page and have it turned it 24 hours prior to that contest. Athletes will not be allowed to leave with anyone other than their parent/guardian if this form is not submitted and approved by the athletic director.
