Travel Permission Form
If you would like your student athlete to ride home with someone other than a parent/guardian after a game, please fill out the form on this page and have it turned it 24 hours prior to that contest. Students will not be allowed to leave with anyone other than a parent/guardian if this form is not signed and approved by the athletic director.
