Every athlete must complete the 2023-24 online participation forms through Rank One. Please go to the website listed below to fill out all required forms. If you need a hard copy of the physical or asthma action planform, please click on the "Download and Print" button to obtain a copy of these forms.
Once you click the link, proceed to clicking on Cole Athletic Form. From there, you will need to complete every section on the form. Athletes will need to know their Student ID number.
